Affidavit of Intent to Home School
This is a general Affidavit of Intent to Home School for the state of Arizona. Please note, certain County School Superintendent's offices may request that you use their version of the Affidavit. This form is generally accepted by most counties and contains all information that we are required by law to submit to the County School Superintendent's office.
Home School Pupil Withdrawal Form
This form is intended to notify the County School Superintendent within 30 days of when your child is no longer home schooled. Provided by the Maricopa County School District.
