Home School Change of Information Form
This form is used for address changes and to update information on your studen'ts affidavit of intent. Provided by the Maricopa County School District.
Affidavit of Intent to Home School
This is a general Affidavit of Intent to Home School for the state of Arizona. Please note, certain County School Superintendent's offices may request that you use their version of the Affidavit. This form is generally accepted by most counties and contains all information that we are required by law to submit to the County School Superintendent's office.
